I agree and consent to the following:

I am voluntarily participating in the exercise/fitness/workout/open gym/ TOURNAMANET at the TJ Robinson Life Center/Robinson Courts Inc or Robinson Real Estate LLC. My players, parents and organization recognize that the program requires physical exertion that may be strenuous at times and may cause physical injury and I am fully aware of the risks and hazards involved.

I understand that it is my responsibility to ensure my players consult with a physician before and regarding my participation in the above-mentioned activity. I represent and warrant that they nor myself have no medical condition that would prevent my participation in the activities.

I agree to assume full responsibility for any risks, injuries or damage know or unknown which my players, parents and organization might incur as a result of participating in the mentioned activities Such injuries may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, injuries to knees, injuries to the back, injuries to the foot, or any other illness or soreness, including death.

My players, parents and organization knowingly, voluntarily, and expressly waive any claim I may have against TJ Robinson Life Center/Robinson Courts Inc or Robinson Real Estate LLC. for injury or damages that I may sustain as a result of participating in activities.

My players, parents and organization forever release waive, discharge, and covenant not to sue the TJ Robinson Life Center, Robinson Courts Inc, or Robinson Real Estate LLC for any injury or death caused by their negligence or other acts. I have read the above waiver and release of liability and fully understand its contents. My players, parents and organization voluntarily agree to the terms and conditions stated above. I hereby give permission for images of My players, parents and organization, captured during Participation of Life Center/Robinson Courts/Robinson Real Estate LLC. Fayetteville Stars Sports events through video, photo and digital camera, to be used solely for the purposes of( Life Center/Robinson Courts/Robinson Real Estate LLC. Fayetteville Stars) promotional material and publications, and waive any rights of compensation or ownership thereto.

Please note that there is no outside food/drinks, no bags, no outside shoes on the courts, and, no profanity upon entry. Failure to follow these rules will result in one-time warning. Continued refusal to follow the rules will result in temporary and/or permanent removal from the facility.
